Based in Launceston, the brand-new Healthcare Connect North service (part of Primary Health North, Tasmanian Health Service) is looking for an experienced General Practitioner to lead the team.

The service will provide intensive multidisciplinary complex chronic conditions care with the goal of supporting high users of hospital care to remain in the community. Community-based, the service will support local GPs and services with no out-of-pocket cost for clients. 

This role will provide the successful applicant with the opportunity to make a real difference to the health outcomes of this group of clients by providing medical supervision and support to a highly skilled multi-disciplinary team. The service will be implemented with the full support of the hospital system and community services as required. The outcomes of the service will be comprehensively evaluated and will contribute to the growing evidence base of innovative ways of working to improve health outcomes for this groups of clients.

What’s on offer?

• Full-time position from 27 June 2022 until 28 June 2024
• Salary range $159,262 - $242,079 per annum depending on qualifications and experience
